Fentanyl patch uses a factor of 3.6 (12 µg/h ≈ 43 mg oral morphine/24 h, the upper part of the guide’s 30–45 mg range); sublingual buprenorphine (8–16 mg) uses the mid-point. Tally adds entries into a single total rather than converting them. Converting that total applies the reduction to every opioid when the tally mixed molecules; if every entry was the same molecule, that molecule keeps its route-change exemption.
